The Consumer Support Grant (CSG) Program is a state-funded alternative to Medicaid reimbursed home care. Eligible participants receive monthly cash grants to replace fee-for-service Home Care payments and, with county assistance, manage and pay for a variety of Home and Community Based Services.

Eligibility criteria:
Participants must be recipients of or eligible for Medical Assistance, must have functional limitations requiring ongoing supports to live in the community, live in a natural home setting, be able to direct and purchase their own supports or have an authorized representative act on their behalf. Consumers cannot concurrently receive the CSG grant and coverage through the state’s managed care program or a Home and Community Based Service Waiver.

Additional notes:
Can be used for special equipment and transportation. All other available sources of payment must be exhausted before use of the CSG.
