The Family Support Group Grant Program provides cash grants to eligible families for the purchase of services and items, which can include assistive technology, necessary to maintain children with developmental disabilities in the family home.

Ages Served:
School age 0-22 years (the month they turn 22)

Types of disabilities served:
Developmental disabilities. Persons with mental retardation or related conditions as determined by the county of residence

Area of Minnesota served:
Statewide

Eligibility criteria:
Families are eligible who have children under age 22, biological or adopted, with mental retardation or a related condition. The child must be determined by case manager to be “at risk” for out-of-home placement to a licensed residential program. Grants must be used to purchase services and items that are unavailable through other funding sources and are directly related to the disabling conditions. Families receiving Title 19 home based waiver are not eligible. Family income must not exceed $75,122 annually.

Maximum amount individual can receive:
$250 per month.
